If you wish to register with the practice, you should bring your signed medical card to reception. If you do not have a medical card, the receptionist will provide the appropriate registration form.
When a patient is registered, they will receive an appointment for a New Patient Health Check with the Practice Nurse. It would be helpful if the patient could provide a list of current medication, immunisations (babies red book) allergies and any other relevant information.
